We’re way past a spate.

1990 and 1991: two separate unidentified NYPD officers (per NYPD Police Academy Firearms and Tactics Section reports)
…and, to my mind, the all-time winner…
May or June of 1998: Chief Richard Williams of Madison, WI, hid his gat in his microwave oven, then later “absent-mindedly started roasting a turkey” (how do you miss noticing a gun on the turntable?!), sending a round out into his house.
